The following items are required during the registration period:

1.    A completed application form
2.    A recent passport sized photo
3.    A copy of the applicant’s immunization record along with a completed medical report
4.    A copy of the applicant’s birth certificate and NIB card
5.    Registration fee of $20.00 (Non-refundable)
